What is Tip Rhinoplasty?

Tip rhinoplasty, medically termed "tipplasty," is a surgical procedure specifically targeting the lower third of the nose — the nasal tip — without touching the nasal bones. While a full rhinoplasty addresses the entire nose (bones, bridge, and tip together), tip rhinoplasty is confined to reshaping the lower lateral cartilages, the columellar strut, and the fibro-fatty tissue of the nasal tip. Because no osteotomies (bone cuts) are performed, the procedure is substantially less invasive, associated with less swelling and bruising, and allows a considerably faster recovery than full rhinoplasty.

In Which Cases is Tip Rhinoplasty Performed?

  • Drooping Nasal Tip: When the tip points downward at rest or drops further when smiling, affecting the overall facial aesthetic balance.
  • Bulbous or Round Nasal Tip: A wide, round, or undefined tip caused by large or poorly shaped lower lateral cartilages and excessive fibro-fatty tissue.
  • Asymmetric Nostrils or Nasal Tip: One nostril is noticeably larger or positioned differently from the other, creating facial asymmetry.
  • Pinched Nasal Tip: A tip that appears unnaturally narrow or pinched, sometimes from previous surgery, with weakened cartilage structure.
  • Tip Projection Issues: A tip that projects too far from the face (over-projection) or does not project enough (under-projection), creating imbalance with the rest of the nose and face.

How Does Tip Rhinoplasty Differ from Full Rhinoplasty?

The critical difference is scope. In full rhinoplasty, nasal bones are cut and repositioned (osteotomies), the dorsal bridge is raised or lowered, and the tip is reshaped — all in a single operation. Tip rhinoplasty addresses only the tip cartilages. This means no bone fracture, no dorsal hump reduction, and no significant impact on the nasal bridge height. For patients whose nasal bridge is proportionate but whose tip is the sole concern, tip rhinoplasty is the more targeted and efficient solution. Patients with both a hump and a tip complaint require full rhinoplasty.

Surgical Technique

Tip rhinoplasty can be performed through either the open or closed approach, depending on the complexity of the correction required. In the open technique, a small columellar incision provides direct access to the tip cartilages. In the closed technique, all work is performed through intra-nasal incisions. The surgeon reshapes the lower lateral cartilages using precise suturing techniques (tip sutures), removes excess fibro-fatty tissue where needed, and may add a small columellar strut graft for improved tip support and projection. The goal is a defined, natural-looking tip that integrates seamlessly with the rest of the face.

Recovery Process

Because no bone work is involved, the recovery from tip rhinoplasty is noticeably faster and more comfortable than full rhinoplasty. A small nasal bandage or taping is applied for approximately 5–7 days. Bruising is minimal or absent, and swelling is limited to the nasal tip area. Most patients comfortably return to desk work and social activities within 1 week. The nasal tip continues to refine as residual swelling resolves, with the final result becoming fully apparent at around 6–9 months.

Tip rhinoplasty can be performed under either general or local anesthesia with sedation, depending on the surgeon's preference and the complexity of the case. General anesthesia is more commonly used for comfort and precision.

Will tip rhinoplasty affect my nasal breathing?

Tip rhinoplasty does not alter the internal nasal airway structures. When performed by an experienced surgeon who preserves adequate cartilage support in the nasal valve area, breathing is unaffected and may even improve slightly if a drooping tip was previously narrowing the internal valve.

Is the result permanent?

Yes. The structural changes made to the cartilage are permanent. However, the natural aging process continues after surgery, and the tip may show very gradual changes over many years as it would have without surgery.
